Consent Agreement for collecting Feedback(UAE)

This Consent Agreement is provided to inform that the Customer has given the consent to the Service provider (“Practo”) for collecting feedback.

WHEREAS:

  1. Practo operates a healthcare platform (“Platform”) accessible at “www.practo.com” that facilitates online appointment booking, feedback collection, and other healthcare-related services for listed doctors and healthcare providers.
  2. The Customer uses Insta HMS software to manage patient data, visits, and appointments across its healthcare centers.
  3. The Customer wishes to enable Practo to access patient visit and appointment data from the Insta HMS software for the purpose of enhancing the feedback process on Practo’s Platform.

NOW, THEREFORE, in consideration of the mutual covenants and agreements contained herein, the parties agree as follows

1. Grant of Access and Consent

The Customer hereby grants Practo access to the patient visit and appointment data stored within the Insta HMS software for the purpose of collecting feedback for doctors listed on “www.practo.com”.

2. Data Usage and Confidentiality:

2.1 Permitted Use:The patient data accessed by Practo under this Agreement shall be used exclusively for feedback collection purposes and shall not be used for any commercial, promotional, or marketing activities.

2.2 Confidentiality Commitment: Practo is committed to maintaining the confidentiality and privacy of patient information in accordance with applicable data protection regulations. The data shall be handled with the highest standards of security and confidentiality.

2.3 No Disclosure: Practo shall not disclose, share, or otherwise distribute patient data to any third party except as necessary to comply with applicable laws or legal processes.

3. Data Privacy and Security:

3.1 Data Protection Compliance:Practo agrees to comply with all applicable data protection laws and any relevant local data privacy laws.

3.2 Data Security:Practo shall implement appropriate technical and organizational measures to protect patient data against unauthorized access, loss, or disclosure.

4. Termination and Revocation of Consent:

4.1 This Agreement shall remain in effect until terminated by either party with thirty (30) days’ written notice.

4.2 Upon termination, Practo shall cease accessing patient data from Insta HMS and ensure that any data obtained under this Agreement is deleted in accordance with applicable laws and regulations.

5. Governing Law

5.1 This Agreement shall be governed by and construed in accordance with the laws of UAE, without regard to its conflict of law principles.
